Enter the email address where you received instructions to access these settlement documents and your Date of Birth (MM/DD/YYYY), and then click the “Continue” button. Note: If you are a Representative Claimant for a deceased Claimant, you must enter the Claimant’s Date of Birth and not your own.
If the email address and Date of Birth match our records, you automatically will receive a separate email containing a secure six-digit access code to enter on the next screen.
